The first 24-48 hours are crucial after a hospital and/or rehabilitation stay. People are at greater risk for falling, poor nutritional intake, dehydration, weakness, and non-compliance with treatment, which results in re-hospitalization. That is where we come in, providing services to ease the transition.

 

The following are a list of programs we provide to help you transition safely: (Minimum of 4 hours)

  • Assisting with transportation home

  • Medication reconciliation (by nurse) and medication Pick-up at pharmacy

  • Unpacking

  • Grocery shopping and meal preparation

  • Bathing and toileting

  • Light housekeeping/laundry/linen change

  • Transport/escort to follow-up doctor’s appointments
