Nurse Practitioner

Home health nurse practitioners are advanced practice nurses who serve as primary health care providers for patients requiring health care in the home setting.  Home health nurse practitioners have multiple roles including patient education, coordination of services and referrals to community resources for the homebound.


  • Sick and Wellness visits

  • Perform a comprehensive initial assessment for newly enrolled patients.

  • Manage patient care on a longitudinal basis, with the primary objective of delivering proactive chronic disease care and significantly reducing avoidable hospitalizations.

  • Diagnose, prescribe and treat.

  • Manage chronic disease.
